Family Building Options for Lesbian Couples
For couples assigned female at birth, there are several routes to parenthood:
- Intrauterine Insemination (IUI) with Donor Sperm
A common starting point, IUI involves inserting donor sperm into the uterus during ovulation. Sperm may come from an anonymous donor or a known donor. - Reciprocal IVF
Also called co-maternity, reciprocal IVF allows both partners to participate biologically: one provides the eggs, and the other carries the pregnancy. This option offers a meaningful way to share the journey of conception and parenthood. - In Vitro Fertilization (IVF) with Donor Sperm
Some couples may choose IVF over IUI due to fertility challenges, age, or a desire for embryo testing (PGT).
Family Building Options for Gay Male Couples
For couples assigned male at birth, the most common option is gestational surrogacy:
- IVF with an Egg Donor and Gestational Carrier
An egg donor provides eggs, which are fertilized with one or both partners’ sperm. A gestational carrier (surrogate) then carries the pregnancy. This option allows for a genetic connection to one or both partners, depending on the setup. - Sperm Donation to a Known Egg Donor and Surrogate
Some couples prefer to use a known egg donor (a friend or relative) and/or a known gestational carrier to maintain a connection or for personal, ethical, or legal reasons.
Options for Transgender Individuals and Nonbinary People
Family planning for transgender and nonbinary individuals may include fertility preservation (such as egg or sperm freezing before medical transition) or assisted reproduction tailored to current reproductive anatomy.
